New Client Forms

If you will be boarding with me, please complete the document below which consists of my New Client Form and Boarding Agreement. Please email the completed forms to me at jtrainingpaws@gmail.com prior to the boarding stay.
Please Note: While I do my best to provide photo updates during your dog’s stay, I care for multiple dogs at a time and I‘m unable to provide daily updates. Thank you for understanding as I prioritize hands-on care and safety throughout each day. In general, you can expect:

• 1 update for 1-3 day stays
• 2 updates for 4-6 day stays
• Additional updates may be provided for extended stays (over 7 days)

Updates are typically sent in the evenings after 7 PM and are spaced out evenly throughout the stay, for example if 6 night stay, updates will be sent on the evening of day 3 and 6.
